Duties:
- Provide patient care and treatment in accordance with the physical therapy plan of care
- Conduct evaluations and assessments to determine appropriate treatment plans
- Develop and implement individualized treatment programs for patients
- Utilize knowledge of physiology and anatomy to address patient needs
- Perform trigger point therapy and other specialized techniques as needed
- Collaborate with other healthcare professionals to coordinate patient care
- Document patient progress and maintain accurate records
- Educate patients and their families on exercises and techniques for at-home care
Skills:
- Strong knowledge of physiology, anatomy, and medical terminology
- Ability to perform trigger point therapy and other specialized techniques
- Excellent communication skills to effectively interact with patients, families, and healthcare professionals
- Strong problem-solving skills to address patient needs and adjust treatment plans as necessary
- Ability to work in an acute care setting, such as a hospital or clinic
- Experience working with pediatric patients is a plus
• Counsel patients and their families on what to do following treatment.
• Observe the patients they are working with before, during and after therapy and report notes to the Physical Therapist.
• Assign specific exercises for the patient and help the patients do these exercises.
• Use devices, such as walkers, and other equipment to help patients regain movement and reduce pain.
• Use a variety of techniques, including stretching exercises and massage, to treat patients.
• Assist patients when moving to and from the therapy area.
The Physical Therapist is a member of the home care team who provides rehabilitation care to the clients as requested by the client and, when required, as prescribed by the physician and in compliance with applicable laws and regulations and the policies of the Agency.
- Possesses a degree from a baccalaureate or masters program in physical therapy approved by an accredited organization.
- Community/home health experience is preferred.
Complies with accepted professional standards and practice. - Possesses and maintains good physical stamina and mental health. Has presented a preemployment screening including a negative TB skin test and/or CXR and other tests as required by the Agency.
- Possesses and maintains current CPR certification.
