Introduction

RASTEEN PLLC, doing business as AMP PT (“AMP PT,” “we,” “our,” or “us”), operates the SMS messaging program described in this policy. This Privacy Policy explains how we collect, use, share, and protect your personal information when you choose to receive text messages from AMP PT.

1. Information We Collect

• Phone Number: The mobile number you provide to receive SMS messages.

• Message Interaction Data: Delivery and engagement metrics (e.g., delivery status, response keywords).

• Consent Records: Information documenting your SMS messaging choices, including your signed paper SMS consent form, the date of consent, and any opt-out requests.

2. How We Use Your Information

• Send SMS messages ( appointment reminders, scheduling notifications, and other communications related to your care) per your opt-in preferences.

• Document your consent and messaging preferences and honor opt-out requests.

• Improve messaging and measure engagement.

3. SMS Consent and Mobile Information

Patients opt in to AMP PT’s SMS program by voluntarily checking the SMS consent box and signing our paper SMS consent form. Messages are sent to the mobile phone number the patient provides. Consent to receive text messages is optional and is not a condition of receiving treatment or services.

We do not sell or share mobile phone numbers, SMS opt-in records, or messaging consent with third parties or affiliates for marketing or promotional purposes.

4. Message Frequency & Data Rates

The SMS message frequency will vary but will not be more than 3-5 messages per day unless there is a notification event. Message and data rates may apply; check your carrier’s terms.

5. Opt-Out & HELP Instructions

• Text “STOP” to any message to unsubscribe immediately.

• Text “HELP” for assistance or contact us at admin@ampptdfw.com or 972-232-2227.

6. Data Sharing & Disclosure

We may share information with service providers, including messaging platforms and mobile carriers, only as necessary to deliver and support AMP PT’s SMS program. These providers may use that information only to provide services on our behalf, not for their own marketing or promotional purposes.

We do not sell or share mobile phone numbers, SMS opt-in records, or messaging consent with third parties or affiliates for marketing or promotional purposes. Consent to receive messages from AMP PT is not transferred to another business for its own messaging program.

7. Security of Your Information

Reasonable measures are in place to protect your data, though no system is infallible.

8. Record-Keeping & Proof of Consent

We retain signed paper SMS consent forms and records of opt-out requests to document permission to send messages and honor requests to stop messages.

9. Changes to This Policy

We may update this Privacy Policy from time to time. Updates will be posted on this page with a revised date.

Changes to this policy do not expand the scope of your SMS consent or override a request to stop receiving messages.
