Introduction

RASTEEN PLLC, doing business as AMP PT (“AMP PT,” “we,” “our,” or “us”), operates this SMS messaging program. These Terms and Conditions apply to recurring text messages sent to patients who have opted in to receive messages from AMP PT. Messages may include appointment reminders, scheduling notifications, and other communications related to your care.

1. Opt-In Consent

Your participation in AMP PT’s SMS service requires your explicit consent. By opting in, you consent to receive messages from AMP PT only.

AMP PT does not sell or share mobile phone numbers, SMS opt-in records, or messaging consent with third parties or affiliates for their marketing or promotional use.

Patients enroll during the in-person intake process by providing their mobile phone number, voluntarily checking the SMS consent box, and signing AMP PT’s paper SMS consent form. Consent to receive text messages is optional and is not a condition of receiving treatment or services.

2. Message Frequency

The SMS message frequency will vary but will not be more than 3-5 messages per day unless there is a notification event.

3. Opt-Out Process

Reply “STOP” any time to end messages; “HELP” to receive support instructions; or contact admin@ampptdfw.com / 972-232-2227.

4. Terms of Consent

Your consent is voluntary and not a condition of purchase. You may receive informational messages as described at opt-in.

5. Privacy Policy

Our Privacy Policy explains how RASTEEN PLLC, doing business as AMP PT, collects, uses, shares, and protects information associated with this SMS program.

6. Message & Data Rates

Message and data rates may apply to text messages you send or receive. Charges depend on your mobile carrier and service plan. Contact your mobile carrier with questions about your plan or charges. AMP PT is not liable for carrier charges.

7. Record-Keeping & Compliance

We retain signed paper SMS consent forms and records of opt-out requests to document permission to send messages and honor requests to stop messages.

8. Changes to Terms

We may update these Terms and Conditions from time to time. Updates will be posted on this page with a revised date.

Changes to these Terms do not expand the scope of your SMS consent or override a request to stop receiving messages. We will obtain any additional consent required before sending messages outside the scope of your existing consent.

9. Disclaimers & Liability

Message delivery depends on carrier networks; we cannot guarantee uninterrupted service or delivery times. Carriers are not liable for any delayed or undelivered messages.
