AMP PT obtains consent to send SMS communications directly from patients using the consent form displayed below. Patients voluntarily provide their mobile phone number and sign this form to opt in to SMS communications from AMP PT. Messages may include appointment confirmations, appointment reminders, scheduling and rescheduling communications, and other communications related to their care. Consent to receive SMS messages is optional and is not a condition of receiving healthcare services. Message frequency may vary. Message and data rates may apply. Patients may reply STOP to opt out or HELP for assistance.
