Patient SMS Opt-In Documentation
RASTEEN PLLC, doing business as AMP PT, obtains SMS consent during in-person patient intake. Patients opt in by providing their mobile phone number, voluntarily checking the SMS consent box, and signing the paper SMS consent form displayed below.
Messages may include appointment reminders, scheduling notifications, and other communications related to your care. Message frequency varies but will not exceed 3-5 messages per day unless triggered by a notification event. Message and data rates may apply. Reply HELP for help or STOP to opt out at any time.
Consent to receive text messages is optional and is not a condition of receiving treatment or services. Mobile information will not be shared with third parties or affiliates for marketing or promotional purposes.
